Basically each section is the same. The names run vertical and the standards across the top. I would jot down a quick note under the standard of the assignment, and record the date. Then I would record the score. Here is an example using a 1-4 scale.
And here is an example from my grade book last year. We actually use a 1-3 scale at my school. The Rs you see recorded for a student is for a gifted student who received math instruction from another teacher whose last name started with an R. She was responsible for his math grades so I simply noted that in my gradebook.
This grade book includes all subjects (Math, Reading, Language, and Writing). Each section is separated by a title page that indicates the domain or subject area.

I downloaded the font from the link you gave for the font on the first page of your gradebook in the picture and MTF-jumping-jack does not match what is in the picture. Is there another font that you used??

Question: I assume you assess students on a standard more than one time. Do you have multiple pages of those standards? Or do you stop assessing students once they have mastered a standard? I am working on getting my head around this process and it is hard to get it out of the old ways.
